New Patient Information
When it comes to treating mental health, there is no one-size-fits-all treatment method. With that in mind, we can customize the treatment to the patient’s mental and physical needs. The treatment itself will involve a combination of various treatment methods, therapy and possible medication. We will only recommend medication when necessary. General Information
- Initial appointments last approximately 60 minutes.
- If the patient is a minor (under 18), both the child and a parent/guardian must be present for the initial evaluation appointment.
- Please call at least 24 hours prior if you need to cancel an appointment.
- Please note that if you arrive late for your appointment, you may be asked to reschedule.
- The office will be closed to observe major holidays.
- In case of inclement weather, closings will be announced on the main page of our website and on our Facebook and Instagram pages.
Commercial Insurance
Fortis Behavioral Health collaborates with Headway, a third-party billing company, allowing us to accept insurance plans from Cigna, Aetna, and United Healthcare, and Blue Cross & Blue Shield, including both commercial and state plans.
To streamline our billing process, Headway requires patients to keep a card on file for co-pays. Patients will be automatically billed their co-pay the day of their scheduled appointment.
For those who prefer to self-pay, we are happy to accommodate and will provide an itemized and CPT coded Superbill for submission to your insurance provider, in accordance with their reimbursement policies.
Tricare
Discover peace of mind with our comprehensive acceptance of TRICARE Select, Reserve, and Life plans. Depending on your specific TRICARE plan, you may have a deductible to meet, and you could be billed for co-insurance after your appointment or for your co-pay prior to your visit. We understand the importance of clarity when it comes to insurance coverage, so please consult with us regarding your individual plan details.
North Carolina Medicaid Plans
Fortis Behavioral Health currently accepts NC Medicaid managed care plans. Please consult with us regarding your specific plan details. It’s important to note that Medicaid patients do not have a co-pay for their visits. If you have any questions or need assistance understanding your coverage, feel free to reach out to our team.
North Carolina Medicare Plans
We currently accommodate Medicare Standard Plan and various Medicare Advantage plans. For Medicare patients, billing occurs after appointments due to co-insurance. Be sure to review your plan details to understand the percentage you’re responsible for. Reach out to us today to discuss the specifics of your plan.
Intake Forms
Upon activation of your patient portal, you’ll receive intake forms promptly. We kindly request completing these forms at your earliest convenience before your appointment, allowing ample time for review. In case you encounter any difficulties accessing the portal, we encourage arriving 30 minutes prior to your appointment to complete the forms in our office. Please note, you must complete the forms before being seen.
Medical Records Request
Patients who were referred to Fortis Behavioral Health by their Primary Care Provider or other health provider please have your medical records faxed to our office prior to your visit.
If you have had previous psychiatric care it is important that those records are provided to our office before your initial appointment.
Illness Policy
If you’re experiencing symptoms like fever, cough, fatigue, or body aches, please contact our office. We’ll arrange a telehealth appointment if feasible. Upon arrival, temperatures will be checked. Individuals with temperatures above 99 degrees will be rescheduled and not seen by the provider.
