New Patient Information
When it comes to treating mental health, there is no one-size-fits-all treatment method. With that in mind, we can customize the treatment to the patient’s mental and physical needs. The treatment itself will involve a combination of various treatment methods, therapy and possible medication. We will only recommend medication when necessary. General Information
- Initial appointments last approximately 45 minutes.
- If the patient is a minor (under 18), both the child and a parent/guardian must be present for the initial evaluation appointment.
- Please call at least 24 hours prior if you need to cancel an appointment.
- Please note that if you arrive late for your appointment, you may be asked to reschedule.
- The office will be closed to observe major holidays.
- In case of inclement weather, closings will be announced on the main page of our website and on our Facebook and Instagram pages.

This plan may include medication management, Lab tests, Genetic testings, referrals to external providers, and other necessary interventions. Commercial Insurance
Fortis Behavioral Health accepts a wide range of commercial insurance plans, including Blue Cross Blue Shield, Aetna (commercial and state health plans), Cigna, UnitedHealthcare, UMR, First Carolina Care, Ambetter, Oscar Health, Oxford Health Plans, and more.
Patients with commercial insurance are required to maintain a valid card on file. This allows us to securely process co-pays, co-insurance, and any applicable patient responsibility at the time of service.
For those who prefer to self-pay, we are happy to accommodate. Upon request, we can provide an itemized, CPT-coded superbill for submission to your insurance provider, in accordance with their reimbursement policies.
North Carolina Medicaid Plans
Fortis Behavioral Health currently accepts NC Medicaid managed care plans. Please consult with us regarding your specific plan details. It’s important to note that Medicaid patients do not have a co-pay for their visits. If you have any questions or need assistance understanding your coverage, feel free to reach out to our team.
North Carolina Medicare Plans
We currently accommodate Medicare Standard Plan and various Medicare Advantage plans. For Medicare patients, billing occurs after appointments due to co-insurance. Be sure to review your plan details to understand the percentage you’re responsible for. Reach out to us today to discuss the specifics of your plan.
Intake Forms
After scheduling your appointment, you will receive a patient portal invitation via email. The portal allows you to request prescription refills, message your provider, view upcoming appointments, join telehealth sessions, and complete required forms.
Intake forms are sent via text message approximately one week prior to your scheduled appointment. Please complete these forms as soon as possible to allow adequate time for review.
If you are unable to locate the text message or did not finish the forms, you can log into the patient portal and access them under the “Questionnaires” section.
All intake forms must be completed no later than the day before your initial appointment. Appointments will be rescheduled if forms are not completed in advance.
Medical Records Request
Patients who were referred to Fortis Behavioral Health by their Primary Care Provider or other health provider please have your medical records faxed to our office prior to your visit.
If you have had previous psychiatric care it is important that those records are provided to our office before your initial appointment.
Illness Policy
If you’re experiencing symptoms like fever, cough, fatigue, or body aches, please contact our office. We’ll arrange a telehealth appointment if feasible. Upon arrival, temperatures will be checked. Individuals with temperatures above 99 degrees will be rescheduled and not seen by the provider.
