At Fortis Behavioral Health, we’ve made it simple for new patients to request an appointment.
Use the calendar to select a time that works best for you. Once you’ve submitted your request, you’ll complete a brief pre-screening form so we can better understand your needs.
Please note that this is an appointment request only. Our office will review your screening questions and selected time. We will either confirm your appointment or contact you if we are unable to accommodate your request. Appointment type and provider assignment are determined after review of your screening information.

It depends. Commercial insurance, Tricare, Medicare, and Medicaid have different policies. If you are unsure please contact our office with questions.
