Existing Patient Information
Existing mental health patients are encouraged to regularly review the provided information, as it is updated periodically. Please check back for any changes to ensure you stay informed about your mental health care.
General Information
- Follow up medication appointments last approximately 30 minutes.
- If the patient is a minor (under 18), both the child and a parent/guardian must be present for the initial evaluation appointment.
- Please call at least 24 hours prior if you need to cancel an appointment.
- Please note that if you arrive late for your appointment, you may be asked to reschedule.
- The office will be closed to observe major holidays.
- In case of inclement weather, closings will be announced on the main page of our website and on our social media pages.
Patient Portal
All Patients are required to register for the online patient portal and must provide an active email address. An email invitation to sign up for the portal will be sent to that email address with instructions on how to access the patient portal. Be sure to check your SPAM folder if unable to locate email invitation.
Medication Refill Procedures
Medications are prescribed at appointments and should last between appointments. If there is an emergency or out of town travel scheduled, please contact our office or make a request through the patient portal.
There will be no early refills of controlled substances regardless of cause. There are no exceptions to this policy.
