Please include the patient’s relevant medical history, any completed referral forms, and other supporting documents like test results, and patient contact information.
Once we receive the referral, our team will reach out to the patient within 48-72 hours to schedule an appointment.
The current wait time for a new patient appointment is approximately 3-4 weeks. Please check with our office for the most accurate and up-to-date availability.

Yes, we offer telehealth appointments for most of our services. You can specify this preference when submitting the referral.
Unfortunately, we do not offer STAT or urgent appointments. All patients are seen in the order the referral is received. As a practice, we do not see patients currently in crisis, as this falls outside of our acuity level.
