Self-Pay Rates
Fortis Behavioral Health offers transparent self-pay pricing for patients in Lumberton, North Carolina, and through statewide telehealth services. Self-pay rates apply to visits not billed to insurance; many patients can request a superbill for possible reimbursement through their insurance provider. Rates vary by service type and provider.
*Appointments require a payment method on file which is charged 24 hours before the scheduled session*
Self-pay patients may request a Good Faith Estimate in accordance with federal guidelines.

Accepted Payment Methods

Insurance Participation
Using Insurance at Fortis Behavioral Health
Fortis Behavioral Health participates with select insurance plans. If we are in-network with your insurance, we will submit claims to your insurance company on your behalf.
However, insurance coverage is a contract between you and your insurance company. Patients are responsible for any costs required by their plan, including:
- Copayments
- Deductibles
- Coinsurance
- Non-covered services
If your insurance denies a visit despite proper billing procedures, you may be responsible for the full cost of the visit.
We recommend contacting your insurance company prior to your first visit to confirm:
- Behavioral health coverage
- Deductible status
- Copay or coinsurance amounts
- Telehealth coverage (is applicable)
Payment Responsibility
Payment Responsibility
Payment for services is due after each visit unless other arrangements have been made with the practice.
Fortis Behavioral Health maintains a secure card on file for all patients. This allows us to process your financial responsibility after your insurance has processed your claim.
Your payment method may be charged for:
- Copayments
- Coinsurance
- Deductible amounts
- Self-pay services
- Late cancellation or no-show fees
Payment information is securely tokenized through our payment processor to protect your financial data.
Insurance Claims
Insurance Claims and Patient Balance
Fortis Behavioral Health will submit insurance claims on your behalf when applicable.
After your insurance processes the claim, they will send an Explanation of Benefits (EOB) showing:
- What insurance paid
- What portion is patient responsibility
If a balance remains after insurance payment, the remaining amount will be billed to the patient.
Patients are responsible for any remaining balances not covered by insurance.
Accounts that remain unpaid for 60 days or longer may be referred to a third-party collections agency and may result in restriction of services or discharge from the practice.
Laboratory Services
Laboratory Testing
Your provider may occasionally order laboratory testing to assist with diagnosis, medication monitoring, or treatment planning.
Fortis Behavioral Health does not perform laboratory testing in office.
Laboratory services are performed by independent third-party laboratories such as:
- Labcorp
- Quest Diagnostics
- AccuReference
These laboratories bill your insurance directly and operate independently from our practice.
Depending on your insurance plan, you may be responsible for:
- Deductibles
- Copays
- Coinsurance
If you receive a laboratory bill or have questions about laboratory charges, you must contact the laboratory or your insurance company directly. Fortis Behavioral Health cannot modify laboratory charges.

Patients are responsible for providing accurate and up-to-date information including:
Insurance coverage
Address
Email
Phone number
- Name changes
If insurance information is not current at the time of service, the patient may be responsible for the full cost of the visit.
